Bots can pick up the email domain from email addresses from the web and abuse it. So dont write email addresses in plain text anywhere.

Would they actually have to have access to our SMTP server? I am pretty sure that that's next to impossible in our case (i.e. someone gaining access to it.)

SMTP protocol is really pathetic. It asks you for "real from" field and "displayed from" field differently. Spam detection tools usually filter emails with different "real from" and "displayed from" fields.

It's so weird that you can send emails to some people and display completely different list in "to" field (i.e., the displayed addressee list). When I was a fresh hire, I wrote a small smtp utility that I used to play pranks; I used to send weird emails to colleagues as if being sent by the HR manager with CC list showing CEO and senior management. In reality, those emails were sent by me to a selected group.

However, it's a shame if somebody is really using your SMTP server.
